Ascend Health

We are passionate about empowering people and the community

Team Ellenbrook

Our extensive training means Ascend is able to give health professionals a high level of autonomy, trusting them to decide how best to care for our members. With the assistance of our custom built cloud technology, we've eliminated many of the manual administrative tasks that prevent information flowing between your support workers and your therapist.

Team Ellenbrook covers the suburbs of Aveley - Baskerville - Belhus - Brabham - Brigadoon - Bullsbrook - Cullacabardee - Dayton - Ellenbrook - Gidgegannup - Henley Brook - Herne Hill - Lexia - Melaleuca - Middle Swan - Millendon - Red Hill - The Vines - Upper Swan - Viveash - West Swan - Whiteman - Woodbridge

Meet our team

Our mentors coordinate a team of local support workers to ensure we deliver high-quality support to our mentors. Their role includes training new support workers, communicating with the members in their areas, providing weekly training, and sharing information with out therapists.

Support Workers

Core to Ascend’s unparalleled standards of service is a commitment to employ only university students dedicated to studying health therapies as disability support workers/carers. Our experienced mentors support these dedicated young students to bring a higher standard of care for people with disabilities.
